Substance use during pregnancy: Treatment that protects both lives

Abruptly stopping opioids during pregnancy can cause miscarriage and fetal distress. MAT with buprenorphine or methadone is the standard of care, not abstinence-only approaches.
MAT during pregnancy
Buprenorphine and methadone are safe and recommended. They prevent dangerous withdrawal, reduce illicit use, and improve birth outcomes. Neonatal abstinence syndrome may occur but is manageable.
Alcohol and pregnancy
No known safe amount. Fetal Alcohol Spectrum Disorders cause permanent developmental damage. Treatment should begin immediately.
Stigma
Pregnant women with SUD face severe stigma and sometimes criminal penalties. Compassionate approaches produce far better outcomes for mother and child.
