What is contingency management in addiction treatment?

Contingency management provides tangible rewards for meeting treatment goals, most commonly clean drug tests.

How it works

Patients earn vouchers, prizes, or privileges for verified abstinence. Rewards escalate with consecutive clean tests. A missed or positive test resets the reward schedule. The evidence is strong: CM produces the highest treatment retention and abstinence rates for stimulant use disorders.

Why it works

Addiction hijacks the brain's reward system. CM provides an alternative reward pathway. Immediate positive reinforcement competes with the immediate reward of substance use. It is the only treatment with strong evidence for methamphetamine and cocaine addiction.
