The cost of therapy without insurance is one of the biggest barriers to mental health care. Knowing what to expect — and the strategies available to reduce costs — can help you access treatment even without coverage.

Individual therapy: $100-$300 per session

A standard 50-minute session with a licensed therapist (LCSW, LPC, LMFT, or psychologist) costs $100-$200 in most markets. In high-cost cities (New York, San Francisco, Los Angeles), rates can reach $250-$350. Psychiatrists (who can prescribe medication) charge more — $200-$400 for an initial evaluation and $100-$250 for follow-up medication management visits. Therapy sessions with doctoral-level psychologists typically cost 10-30% more than master's-level therapists. Pre-licensed therapists and graduate student clinics offer significantly lower rates ($30-$80 per session).

Couples and family therapy: $150-$350

Couples and family therapy sessions are typically longer (75-90 minutes) and cost more. Expect $150-$300 per session with a licensed marriage and family therapist. Specialized approaches (Gottman method, Emotionally Focused Therapy) may command higher rates.

How to reduce costs

Sliding scale fees are widely available — many therapists reserve slots for reduced-rate clients. Ask directly: "Do you offer sliding scale?" Open Path Collective charges a one-time $65 membership fee and provides sessions at $30-$80. Community mental health centers provide services on an income-based sliding scale. Graduate school training clinics offer therapy at $10-$40 per session. Group therapy is significantly cheaper than individual — often $40-$80 per session. Telehealth platforms may offer lower rates than in-person practice. Some employer EAPs provide 3-8 free therapy sessions per year.

When cost should not stop you
