Depression treatment centers: When therapy and medication aren't enough

You have tried therapy. You have tried multiple antidepressants. Maybe some helped a little, maybe none helped at all. You are still depressed, and the conventional treatment path feels exhausted. This is not a failure on your part — approximately 30% of people with major depression do not respond adequately to first-line treatments. There are options beyond what most outpatient providers offer.

Residential depression treatment

Residential programs for depression provide 24/7 structured care with intensive daily programming. You receive multiple therapy sessions per day, daily psychiatric monitoring and medication adjustments, structured activities designed to combat the inertia of depression, peer support from others going through similar experiences, and removal from the environment and stressors that may be maintaining the depression. Residential stays typically run 30-90 days and are appropriate when depression is so severe that daily functioning has collapsed.

Advanced treatment modalities

TMS (Transcranial Magnetic Stimulation) is FDA-cleared for treatment-resistant depression with response rates of 50-60%. The newer SAINT protocol can deliver a full course in just 5 days. Ketamine and esketamine (Spravato) offer rapid-acting antidepressant effects, often within hours. Ketamine infusions are available at clinics nationwide; Spravato is administered in certified healthcare settings. ECT (Electroconvulsive Therapy) remains the most effective treatment for severe, treatment-resistant depression, with response rates of 70-90%. Modern ECT is performed under general anesthesia and is far more refined than historical portrayals.

How to evaluate intensive programs

Ask: What percentage of your patients have treatment-resistant depression? Do you offer advanced modalities (TMS, ketamine, ECT) in addition to therapy? How quickly can medication adjustments be made in your program? What is your average length of stay for depression patients? What does your aftercare plan look like to prevent relapse after discharge?
