Free and low-cost rehab options: How to find them in every state
Treatment should never be inaccessible because of money. Free and low-cost treatment options exist in every state — the challenge is knowing where to find them and how to access them.
State-funded treatment programs
Every state operates substance use and mental health treatment programs funded through federal SAMHSA block grants and state budgets. These programs serve uninsured and underinsured residents at no cost or on a sliding scale based on income. To find your state's programs, call SAMHSA's helpline at 1-800-662-4357 and ask to be connected with your state's substance abuse authority. You can also search our directory and filter by facilities that accept Medicaid and sliding-scale fees.
Federally Qualified Health Centers (FQHCs)
FQHCs are community-based healthcare organizations that provide services on a sliding fee scale based on ability to pay. Many FQHCs now offer behavioral health services including substance use treatment, mental health counseling, and medication management. Find one at findahealthcenter.hrsa.gov. You cannot be turned away for inability to pay.
Medicaid
If you earn up to 138% of the federal poverty level in an expansion state (40 states as of 2026), you qualify for Medicaid, which covers substance use and mental health treatment as essential health benefits. Enrollment is available year-round (not just during open enrollment). Apply at healthcare.gov or your state Medicaid office. Many people who qualify do not realize they are eligible.
Nonprofit and faith-based programs
The Salvation Army Adult Rehabilitation Centers operate free residential treatment programs in 130+ locations nationwide. Many churches and faith-based organizations run free recovery programs and support groups. Local nonprofits in your area may operate treatment facilities funded by charitable donations and community grants. Oxford Houses provide self-supporting, self-governed sober living environments at cost (approximately $400-$600/month, shared among residents).
VA benefits
Veterans are eligible for substance use and mental health treatment through the VA system, regardless of service-connected disability status. Treatment is available at VA medical centers and through community care referrals. Contact your local VA or call the Veterans Crisis Line at 1-800-273-8255 (press 1).
