Medi-Cal: Comprehensive free treatment
If you qualify for Medi-Cal (California's Medicaid), addiction treatment is covered at no cost. This includes medical detox, residential treatment, outpatient, IOP, MAT (Suboxone, methadone), and recovery support services. Orange County operates under the Drug Medi-Cal Organized Delivery System (DMC-ODS), which means the county coordinates your care through a managed system.

Who qualifies for Medi-Cal?
In California, Medi-Cal income limits for a single adult are approximately $1,677/month ($20,121/year). For a family of four: approximately $3,450/month ($41,400/year). If you have lost your job due to addiction, if you are unhoused, or if your income has dropped, you likely qualify. Undocumented immigrants in California are now eligible for full Medi-Cal coverage. OC Health Care Agency programs
The OC Health Care Agency operates directly funded substance use disorder treatment programs including outpatient counseling, IOP, withdrawal management (both social model and medical detox), residential treatment, perinatal programs for pregnant women, methadone maintenance and other MAT, recovery residences, and drug court/DUI court services. These programs serve people regardless of insurance status.
Other free and low-cost options
Salvation Army: Free residential treatment (work-therapy model, 6+ months). Community health centers: Federally qualified health centers in OC offer sliding-scale behavioral health services. Veterans: VA Long Beach Healthcare System provides addiction treatment for eligible veterans. Faith-based programs: Several OC churches and organizations offer free recovery support. What free treatment actually looks like
