Ketamine therapy has moved from experimental curiosity to established treatment option for depression that has not responded to traditional antidepressants. What is established
Esketamine (Spravato) is FDA-approved for treatment-resistant depression and major depressive disorder with suicidal ideation. It is administered as a nasal spray in a certified provider's office (you cannot take it home). Sessions occur twice weekly initially, then weekly, then less frequently. You must remain at the provider's office for 2 hours after administration for monitoring. Most insurance covers Spravato when criteria are met (typically two failed antidepressant trials).
IV ketamine infusions for depression are supported by substantial evidence but are administered off-label (not FDA-approved for this indication). Multiple controlled trials show rapid antidepressant effects, often within hours. A typical course involves 6 infusions over 2-3 weeks, followed by maintenance infusions as needed. IV ketamine is typically not covered by insurance and costs $400-$800 per infusion.
What is emerging
Ketamine-Assisted Psychotherapy (KAP) combines sub-anesthetic ketamine with psychotherapy, using ketamine's neuroplasticity-enhancing effects to deepen therapeutic work. This approach is growing in OC but has less controlled trial evidence than standalone ketamine or esketamine. Oral ketamine prescriptions for at-home use have become available through some telehealth providers, but this approach is controversial due to abuse potential and lack of supervision.
