Questions to ask a clinical director before admitting a loved one
The clinical director sets the tone for a treatment program. Their answers to these questions will tell you more about the facility's quality than any brochure or website.
About their clinical approach
What evidence-based modalities does your program use? (They should name specific approaches — CBT, DBT, EMDR, MAT — not just "holistic healing.") How are treatment plans individualized? What is the typical treatment plan revision process? How do you measure patient outcomes? Can you share any outcome data? A clinical director who cannot articulate specific modalities or describe an outcome measurement system is a concern.
About their staff
What are the credentials of your clinical staff? What is the therapist-to-patient ratio? How many individual therapy sessions does each patient receive per week? What is your staff turnover rate? (High turnover is one of the strongest indicators of organizational problems.) Do you have a medical director on staff? How is psychiatric care delivered?
About difficult situations
What happens if a patient relapses during treatment? What is your policy on involuntary discharge? How do you handle psychiatric emergencies? What happens if a patient is a danger to themselves or others? How do you handle patient grievances? These questions reveal how the facility manages real-world clinical complexity — not just the ideal scenario.
About aftercare
What does your discharge planning process look like? When does aftercare planning begin? Do you have relationships with outpatient providers for warm handoffs? Do you offer alumni programming? What is your follow-up process after discharge? A clinical director who talks about aftercare with the same specificity as admission is running a program focused on outcomes, not just occupancy.
