Cost is the number one stated barrier to treatment on r/addiction. And the tragedy is that most people who say they cannot afford rehab do not know about the free options that exist in every state. Here is every pathway to free or low-cost treatment.
Medicaid: Free treatment in every state
Medicaid covers full addiction treatment including detox, residential, outpatient, and MAT. If your income is low enough to qualify (and many people in active addiction qualify), Medicaid is your most comprehensive free option. Apply at healthcare.gov or your state Medicaid office. Many facilities help with Medicaid applications during intake. The community emphasizes: apply even if you think you make too much. Income thresholds are higher than many expect, especially in Medicaid expansion states.
SAMHSA state-funded programs
Every state receives federal block grant funding for substance abuse treatment. These funds support free treatment for people who cannot pay. Call SAMHSA at 1-800-662-4357 and ask specifically for state-funded programs in your area. Wait times vary but free beds exist.
Salvation Army Adult Rehabilitation Centers
Free residential treatment, typically 6-12 months. Work-therapy model where you work in Salvation Army thrift stores during treatment. The Reddit community is mixed on these programs: the free aspect is valued, but the religious component and work requirement are not for everyone. For people with absolutely no resources, Salvation Army is often the most accessible option.
Other free options
Faith-based programs (Teen Challenge, Celebrate Recovery residential): free, donation-funded, varying degrees of religious requirement. Community health centers with sliding-scale fees. University training clinics. VA programs for eligible veterans. Pro-bono beds at private facilities (ask about scholarship programs). Drug court diversion programs.
Your insurance may cover more than you think
The Mental Health Parity Act requires insurance to cover addiction treatment equally with medical care. Many people with insurance do not realize their plan covers residential treatment. Call the behavioral health number on your card and ask specifically about substance abuse benefits. Most treatment facilities verify insurance for free.
