Rural addiction treatment: Finding help when there is no rehab near you
Approximately 60% of rural Americans live in areas designated as mental health professional shortage areas. If you live in a rural community, the nearest treatment facility may be hours away. This does not mean treatment is inaccessible — but it requires creative navigation of the options available.
Telehealth: The biggest equalizer
Telehealth has transformed rural treatment access. MAT providers like Bicycle Health, Workit Health, and groups of addiction medicine physicians offer telehealth buprenorphine prescribing — meaning you can receive evidence-based opioid treatment from your home in a town with zero addiction specialists. Many telehealth therapy platforms accept insurance and provide CBT, trauma therapy, and addiction counseling via video. During and after the COVID-19 public health emergency, regulations were relaxed to allow initial MAT prescribing via telehealth, and many of these flexibilities have been made permanent.
Hub-and-spoke models
Some states have implemented hub-and-spoke treatment systems where a central treatment hub (usually an opioid treatment program in a city) supports local spokes (primary care providers in rural areas) in delivering MAT. Your local family doctor may be able to prescribe buprenorphine with remote support from an addiction medicine specialist — ask your primary care provider if they participate in any such program.
When you need residential treatment
If outpatient options are insufficient and you need residential care, you may need to travel. Consider this an investment, not a barrier. Many treatment facilities offer transportation assistance. Some states fund travel for residents seeking treatment unavailable locally. Leaving your environment can actually benefit recovery by removing proximity to triggers and using people.
