Why fentanyl signs are harder to detect

Many fentanyl users do not know they are taking fentanyl. It is present in counterfeit pills, heroin, and even cocaine and meth. The signs overlap with general opioid use.

Physical signs

Extreme drowsiness or nodding off, pinpoint pupils, slowed or shallow breathing, blue-tinged lips or fingertips, confusion, nausea, and constipation.

Behavioral signs

Small pressed pills that may look like prescription medications, aluminum foil with burn marks, rapid escalation of use, financial desperation, and extreme secrecy.

The critical difference

Fentanyl is 50-100x more potent than morphine. The margin between a high and an overdose is razor thin. If someone is using any street drugs, they may be exposed to fentanyl whether they know it or not. Have naloxone (Narcan) accessible.

Frequently asked questions

How do I know if someone is using fentanyl?
Signs are similar to other opioids: pinpoint pupils, drowsiness, slowed breathing. Many users do not know they are using fentanyl as it contaminates many street drugs.
Can fentanyl kill you the first time?
Yes. Because fentanyl is extremely potent and dosing in street drugs is inconsistent, even first-time exposure can be lethal.
Should I carry Narcan if someone I know uses drugs?
Yes. Narcan is available over the counter and could save their life. Any street drug may contain fentanyl regardless of what it is sold as.
