Where telehealth performs equally

Where in-person may be better

In-person therapy has advantages for severe psychiatric conditions requiring close observation, situations where safety assessment is critical (suicidal ideation, psychosis), group therapy (group dynamics are harder to establish virtually), substance use treatment where environmental control matters, conditions involving body-based work (somatic experiencing, certain trauma modalities), and patients who lack private space for confidential sessions at home.

The practical advantages of telehealth

Beyond clinical equivalence, telehealth eliminates transportation barriers, reduces wait times (telehealth providers often have faster availability), provides access to specialists not available locally, accommodates work and family schedules more flexibly, and reduces stigma for people uncomfortable entering a clinic. For people in rural areas, telehealth may be the only realistic way to access specialized care.

How to decide

If your condition and modality are well-suited to telehealth, convenience and access may tip the scale. If you need higher levels of care, body-based therapies, or group work, in-person is likely better. Many people use a hybrid approach — regular telehealth sessions with periodic in-person check-ins.
