Aftercare planning best practices for treatment centers
Aftercare planning is one of the strongest predictors of post-treatment outcomes. It should begin at admission, not the week before discharge.
Start at admission
Identify aftercare needs during the initial assessment: housing stability, employment, insurance continuation, family support, co-occurring conditions requiring ongoing treatment, and recovery community connections.
Essential aftercare components
Step-down treatment (residential to PHP to IOP to outpatient). Ongoing therapy appointment scheduled before discharge. Recovery meeting connections established during treatment. MAT continuation plan with prescriber identified. Sober living arrangement if home environment is unsafe. Employment or vocational support. Family therapy continuation.
The first 48 hours
The first 48 hours after discharge are the highest-risk period. Schedule an outpatient appointment within 48 hours. Ensure medications are filled before discharge. Have the patient attend a recovery meeting within 24 hours. Confirm safe transportation and housing.
Follow-up
Contact patients at 24 hours, 1 week, 30 days, and 90 days post-discharge. Address barriers to aftercare engagement. Track outcomes to improve aftercare planning for future patients.
