The truth about "free" referral helplines for treatment

When you search for treatment help online, you will encounter many phone numbers offering "free, confidential help." Some are legitimate government or nonprofit services. Others are lead generation operations that sell your call to the highest-bidding treatment center. Knowing the difference can protect you from being steered into a facility chosen for profit rather than fit.

Legitimate helplines

SAMHSA National Helpline (1-800-662-4357): Free, confidential, 24/7 referral service operated by the federal government. They do not sell leads. 988 Suicide and Crisis Lifeline (call or text 988): Government-operated crisis line. Crisis Text Line (text HOME to 741741): Nonprofit crisis service. Veterans Crisis Line (1-800-273-8255, press 1): Government-operated. These services connect you with local resources based on your needs, not based on who pays the most.

Lead generation operations

Many websites and phone numbers that appear to be helpful resources are actually call centers that sell leads to treatment facilities. Warning signs include websites with prominent phone numbers but little substantive content, operators who immediately ask about your insurance before asking about your needs, being transferred to a specific facility without being offered options, and URLs that mimic government sites but are actually commercial operations. These operations are not necessarily illegal (though some violate EKRA), but they introduce a financial conflict of interest into what should be a clinical recommendation.

How to protect yourself

When calling any helpline, ask directly: "Are you operated by a government agency or nonprofit?" and "Do you receive payment from the facilities you recommend?" Use SAMHSA's helpline as your primary resource — it is taxpayer-funded, has no financial conflicts, and provides referrals to all types of treatment in your area. If you use a commercial referral service, treat their recommendation as one data point and independently verify any facility they suggest.
