Why aftercare matters

The first 90 days after residential treatment have the highest relapse risk. You are returning to the environment, relationships, and triggers that contributed to your addiction, now without the structure and support of the treatment facility.

Step-down options

PHP (4-6 weeks of full-day programming). IOP (8-12 weeks of evening sessions). Standard outpatient therapy (weekly). Sober living (3-12 months of supported housing). The specific combination depends on your clinical needs and circumstances.

Building your aftercare plan

Ongoing therapy (individual and/or group). Recovery meeting attendance (AA, NA, SMART). Sober support network. Medication management if on MAT. Employment or education plan. Exercise routine. Relapse prevention plan with identified triggers and responses.

The critical actions

Attend your first recovery meeting within 48 hours of discharge. Fill prescriptions immediately. Set up outpatient appointments before you leave residential. Tell your support network your discharge date. Have a sober living arrangement if home environment is risky.
