United Healthcare (UHC) covers substance abuse treatment through its Optum behavioral health division. UHC is the largest health insurer in the US.

Covered services

Medical detox. Residential treatment (prior authorization required). PHP and IOP. Outpatient therapy and MAT. Psychiatric services.

Optum behavioral health

UHC manages behavioral health benefits through Optum. Call the behavioral health number on your card (not the general number). Optum uses ASAM criteria for level of care determinations.

Important notes

Prior authorization is required for residential and PHP. Utilization review occurs during treatment (the facility handles this). In-network Optum facilities have lower out-of-pocket costs. UHC has been expanding coverage for MAT.
