Medications used in opioid detox

Opioid detox medications range from eliminating withdrawal entirely (MAT) to managing symptoms individually (comfort medications).

Buprenorphine (Suboxone)

Eliminates withdrawal symptoms and can continue as long-term MAT. Gold standard approach. Requires waiting until sufficient withdrawal develops before starting (to avoid precipitated withdrawal).

Methadone

Full agonist that eliminates withdrawal. Dispensed at licensed OTP clinics. Can continue as long-term MAT. No waiting period required.

Clonidine protocol

For patients who decline MAT. Reduces autonomic symptoms (sweating, anxiety, rapid heart rate). Does not eliminate core withdrawal symptoms. Combined with comfort medications.

Comfort medications

Loperamide (diarrhea). Ondansetron (nausea). Trazodone (insomnia). Dicyclomine (abdominal cramping). Ibuprofen (muscle aches). Hydroxyzine (anxiety).
