Gabapentin (Neurontin) has a half-life of 5-7 hours and is eliminated primarily through the kidneys unchanged, meaning liver function has minimal effect on clearance.

Detection

Urine: 2-4 days. Gabapentin is NOT included on standard drug panels. Specialized gabapentin testing is available and increasingly used in treatment and monitoring programs. Blood: 12-24 hours. Hair and saliva testing for gabapentin is not standardized.

Growing testing interest

Several states have reclassified gabapentin as a controlled substance due to abuse potential. Treatment programs and pain management clinics are increasingly testing for gabapentin, particularly when monitoring for polysubstance use.

Clearance factors

Kidney function is the primary determinant since gabapentin is excreted unchanged by the kidneys. Impaired kidney function significantly extends detection. Dose and duration of use matter less than for liver-metabolized drugs.
