Methadone has an exceptionally long half-life of 8-59 hours (average 22 hours), making it detectable far longer than most opioids.
Detection
Urine: 7-14 days. Methadone requires specific testing as it is NOT detected on standard opioid panels. Blood: 1-3 days. Hair: up to 90 days.
Why so long
The long and variable half-life means methadone accumulates with daily dosing. Steady-state levels take 5-7 days to achieve and 5-7 days to clear.
For MAT patients
Positive methadone tests are expected and verify medication compliance. Clinics test to confirm patients are taking their prescribed dose.
