How long does tramadol stay in your system?

Tramadol has a half-life of 5-7 hours with an active metabolite lasting 8-10 hours.

Detection by test type

Urine: 2-4 days with a specific tramadol panel. Important: tramadol is NOT detected on standard opioid panels because it is synthetic. Blood: 12-24 hours. Hair: up to 90 days.

Why standard tests miss tramadol

Standard opioid immunoassays detect morphine and codeine metabolites. Tramadol is structurally different and does not cross-react. Extended panels or specific tramadol assays are required.

Factors

Liver function is critical since tramadol is metabolized by CYP2D6 enzymes. Poor metabolizers (about 7% of Caucasians) clear tramadol more slowly.
